Patients can review health information, lab results, medications, and upcoming visits through the Healow patient portal. For other requests, the medical records team can help by email, fax, or mail.
Use the portal for ongoing access or contact the medical records team for a written request and secure delivery process.
If information is inaccurate or incomplete, you may request a correction. The team will respond in writing and may take up to 60 days to process the request.
You may request a list of certain disclosures made over the last six years. The first request within a 12-month period is free.
You may request bills or other communications be sent to a different location or by a different method to protect your privacy.
You may request limits on how health information is shared for treatment, payment, or operations, subject to applicable rules.

There is no fee for patients accessing records through the portal. Other request types may involve fees depending on the purpose and delivery method. The team should notify requestors before processing if charges apply.
For the fastest response, have the patient name, date of birth, request type, and desired delivery method ready before contacting the team.
